Job description
Elevance Health is seeking a Transitions of Care RN for a 100% virtual role with CareBridge. You will deliver education and disease management interventions for members with chronic diseases, working across multiple lines to improve health outcomes.
The position requires coordination with providers, coaching plans, and motivational interviewing to drive behavior change. Work hours are full-time with weekend coverage; home-based care experience and state RN licensure are required.
