nnnProvide direct patient care in the home

nnMonitor patient condition and report changes

nnAdminister medications and treatments as ordered

nnSupport individualized care plans

nnDocument care and communicate with the healthcare team

n nRequirements
nRequired:
nnnActive LPN license
n
nnCurrent BLS certification
n
n nPreferred:
nn1+ year of nursing experience
n nAdditional Details
