24 - 32 hr RN Case Manager - VNA Home Health Care - Arlington & Belmont Area
VNA Care is hiring a Case Manager RN to join our team for 24-32 hours per week. This position will cover patient visits in the Arlington and Belmont areas.
The ideal candidate will be dedicated to providing high-quality, compassionate care to patients in their homes, ensuring they receive the medical attention, support, and services they need to thrive.
Salary range :
$86,000 - $129,00 for a full-time schedule, would need to adjust for a part time schedule.
Schedules: Work three to five days, 8a-2p or 8a-4:30p, Monday - Friday
Qualifications:
Must hold a valid MA RN License in good standing
Minimum of 1 year of med/surg experience; acute care experience is beneficial
Ability to work with compassion, organization, and excellent multitasking skills
Willingness to travel within a designated territory
Must have Home Health Care experience
Responsibilities
Conduct home visits to assess patient needs and provide necessary care
Utilize Case Management, Oasis E documentation, Time Management, and EMR system, Home Care Home Base
Collaborate with patients, families, and interdisciplinary team members to develop and implement care plans
Ensure accurate documentation of patient progress and treatment plans
