Job description
A community-focused healthcare organization in Fremont, California, is seeking an experienced, Spanish-speaking Licensed Clinical Social Worker (LCSW) to provide integrated behavioral health services and clinical consultation within a primary care setting.
Position Highlights:
In office role, no remote
Provide integrated behavioral health services within a primary care setting
Consult with primary care providers regarding mental health, substance use, and psychosocial concerns
Support patients with behavioral health needs related to chronic pain and chronic disease
Conduct behavioral health screening, triage, evaluation, and assessment
Provide individual, family, and group interventions, including CBT and contingency management
Provide crisis intervention and behavioral health support
Offer group treatment, psychoeducation, and health education
Collaborate with a psychiatrist regarding psychopharmacological consultation within scope of practice
Work closely with medical case managers and interdisciplinary healthcare providers
Provide education and support to patients and families
Coordinate referrals to appropriate community resources
Provide comprehensive services including intake, treatment planning, counseling, collateral intervention, and outreach
Participate in clinical case conferences, peer review, quality assurance, and program evaluation
Provide behavioral health consultation and training to clinic staff
Maintain accurate clinical documentation, case records, reports, and program materials
Contribute to ongoing development of behavioral health procedures and protocols
Practice in a multicultural, multilingual community health environment
Qualifications:
Master s degree in Social Work/Social Welfare or doctoral degree in Clinical or Counseling Psychology
Current California LCSW or Psychologist license
Current LPCC license
Fluent Spanish-speaking ability highly desired
One year of directly related experience preferred
Experience in primary care consultation, behavioral medicine, health psychology, or related settings preferred
Community health experience and/or experience serving low-income multicultural populations preferred
Strong clinical judgment, problem-solving, communication, and collaboration skills
Commitment to culturally and linguistically sensitive care
Ability to work independently and effectively as part of a multidisciplinary team
Willingness to work a flexible schedule as needed
Compensation & Benefits:
$92,200 $106,878.24 annual salary
