Job description
Oswego Health in Fulton, NY seeks an ACT Team Family Specialist to provide clinical treatment, psychosocial rehabilitation, and community support designed to promote recovery and improve functioning in work, school, home and social relationships.
Duties include outreach therapy, housing assistance, medication support under supervision, crisis counseling, and leading family psycho-education groups, plus coordinating with a multi-disciplinary team.
