nPosition Overview
nWe are seeking a highly skilled and compassionate Nurse Practitioner (PMHNP) to join our Assertive Community Treatment (ACT) Team. In this role, you will provide psychiatric evaluation, diagnosis, and medication management to individuals with severe and persistent mental illness in community settings. As a key member of a multidisciplinary team, you will collaborate closely with clinical staff to deliver integrated, recovery-oriented care.
nKey Responsibilities
nnnConduct comprehensive psychiatric evaluations and ongoing assessments in community-based settings
n
nnDiagnose mental health conditions and develop individualized treatment plans
n
nnPrescribe, monitor, and adjust psychiatric medications in accordance with best practices and regulatory standards
n
nnProvide education to clients and families regarding diagnoses, medications, and treatment options
n
nnCollaborate with ACT team members to coordinate holistic care, including medical, behavioral, and social supports
n
nnMonitor client progress, response to treatment, and potential side effects
n
nnParticipate in daily team meetings, case conferences, and care coordination activities
n
nnProvide crisis intervention and clinical support as needed
n
nnMaintain accurate and timely documentation in compliance with agency and regulatory requirements
n
n nQualifications
nnnLicensed Psychiatric Mental Health Nurse Practitioner (PMHNP) in NYS
n
nnMaster's degree in Nursing required
n
nnBoard certification as a Psychiatric Mental Health Nurse Practitioner
n
nnMinimum of 2 years of experience in psychiatric or behavioral health settings
n
nnStrong knowledge of psychiatric disorders, psychopharmacology, and integrated care practices
n
nnExcellent clinical judgment, communication, and interpersonal skills
n
nnAbility to work both independently and collaboratively within a multidisciplinary team
n
n nPreferred Qualifications
nnnExperience working within an ACT team or community-based behavioral health program
n
nnBilingual skills are a plus
n
nnFamiliarity with electronic health records (EHR) systems
n
