Job description
ClearSky Health is seeking a Case Manager for our hospital in Town of Mansfield, New York. The role partners with physicians and a multidisciplinary team to develop and monitor care plans for assigned patients, ensuring progress toward outcomes and active involvement in discharge planning.
This PRN position follows 8 am–5 pm, Monday through Friday, with essential duties including documentation, coordination, accreditation adherence, and effective communication with payors and staff.
