Job description

LanceSoft seeks an experienced Case Manager RN for an acute care hospital setting. The role emphasizes care coordination, discharge planning, patient advocacy, and collaboration with interdisciplinary teams to ensure safe transitions of care.

Ideal candidates will have an active RN/APRN license, an nursing degree, and at least two years of recent acute care Case Management experience. Weekday schedules are available with potential additional weekend shifts.
