Job description
Trinity Health in Fort Lauderdale is seeking an Acute Care Manager to coordinate and manage care across acute, ambulatory and post-acute settings, ensuring handover and continuity while reducing fragmentation of services. This on-site, per diem, part-time role requires an RN with a Bachelor’s degree preferred and 3–5 years of direct patient care or case management.
Strong analytical skills, independent decision making, and collaboration with interprofessional teams are essential; BLS required,
