Job description
Trinity Health Hospital in Grand Rapids, MI seeks an experienced RN Case Manager to coordinate discharge planning and utilization across acute care units. You will assess patient needs, plan care, and collaborate with a multidisciplinary team to optimize outcomes.
This full-time role is day shift, 40 hours per week, with some weekends (approx 6-8 per year) and occasional holidays, emphasizing patient advocacy, care coordination, and evidence-based practice in a Magnet-designated hospital.
