Job description
RN.com is seeking a skilled RN Case Manager in Missouri/Kansas City to coordinate patient care from admission through discharge, ensuring alignment of treatments with each patient’s needs. You will collaborate with physicians, nurses, and external providers to remove barriers to care and facilitate safe, efficient discharge planning.
The role requires MO/compact RN license and BLS, with at least 2 years of case management experience in an acute hospital setting and recent discharge planning
