nAdministrative Case Manager Job Description
nPosition Summary
nThe Administrative Case Manager provides comprehensive administrative case management services in accordance with Single County Authority (SCA) program requirements and DDAP regulations. This role supports individuals with substance use and behavioral health needs through care coordination, referrals, documentation, client engagement, and linkage to community resources. The Administrative Case Manager works collaboratively with providers, physicians, and community stakeholders to ensure clients receive timely and appropriate services.
nEssential Duties and Responsibilities
nnProvide Administrative Case Management services in accordance with SCA program requirements.
nMaintain assigned caseload and ensure all documentation is accurate, complete, and compliant with regulatory standards.
nMaintain at least 60% of work time in direct case management services.
nCommunicate effectively with clients, treatment providers, referral sources, and community stakeholders regarding case progress and service coordination.
nCoordinate care, referrals, and inpatient bed placements for clients in need of treatment services.
nConduct collateral outreach activities to support client engagement and continuity of care.
nLink clients to community and supportive resources including Medicaid (COMPASS), Chesco Connect, housing programs, CRS services, and other case management supports.
nAssist physicians with scheduling appointments, assessments, documentation, and prior authorization processes.
nManage transportation systems and client transportation resources, including Lyft accounts and related scheduling.
nComplete PA WITS data entry accurately and within required timelines.
nEnsure compliance with DDAP regulations, SCA requirements, confidentiality standards, and agency policies.
nAttend supervision sessions, staff meetings, and all required trainings.
nPerform other related duties as assigned.
n nQualifications and Education Requirements
nnMust meet DDAP MET requirements.
nBachelor's degree in a related field with at least one (1) year of relevant experience, or an equivalent combination of education and experience.
nCompletion of required confidentiality, Motivational Interviewing (MI), and DDAP trainings.
nKnowledge of ASAM criteria, substance use disorder systems, and applicable regulations.

nAbility to multitask and prioritize responsibilities in a fast-paced environment.
nExcellent verbal and written communication skills.
nProficiency with EMR systems and PA WITS data entry.
nStrong documentation and recordkeeping abilities.
nAbility to work collaboratively with interdisciplinary teams and community partners.
