Job description
21st Century Home Health Services (21HHS) is seeking an experienced RN to join our field-based team providing care across California. You will conduct Start of Care (SOC) admissions, perform follow-ups, and visit patients in their homes within designated territories, delivering IV therapies and standard wound care per protocol.
