Job description
Home Health & Hospice Care is seeking an Admission RN for the Greater Nashua area to perform initial home health assessments, develop care plans, and coordinate with families and the care team. This full-time, on-site role focuses on delivering skilled nursing care in patients' homes.
Responsibilities include conducting complete physicals, ensuring accurate OASIS scoring, and applying the nursing process to plan, implement, and evaluate patient care with the patient and family actively involved.
