Job description
Transitions Care is seeking an Admissions Registered Nurse to complete hospice admissions, assess patient needs, and initiate individualized plans of care. In this field-based role, you’ll conduct comprehensive assessments, explain hospice services and goals, obtain consents, and coordinate seamless transitions from hospitals, facilities, and homes. Collaborate with an interdisciplinary team to manage symptoms, support families, and ensure dignity and comfort at end of life. Ideal candidates bring strong clinical judgment, excellent communication, and a deep commitment to compassionate, patient- and family-centered care.
