Admissions Registered Nurse (RN) – Home Health Visits

nBAYADA Home Health Care is seeking an experienced Admissions Registered Nurse (RN) for a full-time opportunity with our UCV office , providing home health admissions and OASIS Start of Care (SOC) visits to adult and geriatric clients throughout Monmouth, Middlesex, Union, and Somerset Counties, NJ .

nAs an Admissions RN, you'll play an important role in welcoming new clients to BAYADA and setting them up for a successful home health experience. You'll complete comprehensive initial assessments, establish individualized plans of care, and collaborate with our interdisciplinary team to ensure a smooth transition into home health services.

n nHow You’ll Make an Impact:

nnnComplete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients throughout the assigned territory.

n

nnPerform thorough initial nursing assessments, including medication reconciliation, safety assessments, and evaluation of each client's clinical and functional needs.

n

nnDevelop individualized plans of care in collaboration with physicians, clients, caregivers, and members of the interdisciplinary team.

n

nnIdentify client needs and coordinate appropriate nursing, therapy, social work, and other home health services.

n

nnReview and execute physician orders and ensure the plan of care accurately reflects each client's needs and goals.

n

nnEducate clients and caregivers regarding diagnoses, medications, treatments, safety, and the home health plan of care.

n

nnCommunicate clinical findings and changes in condition to physicians and appropriate members of the BAYADA care team.

n

nnEnsure timely and accurate completion of OASIS assessments and all required admission documentation.

n

nnAccurately document assessments, interventions, education, and care coordination using BAYADA's electronic medical record (EMR) system.

n

nnHelp ensure a smooth transition from referral to admission and ongoing home health services.

n

n nQualifications:

nnnCurrent and unrestricted New Jersey Registered Nurse (RN) license .

n

nnMinimum of one year of recent, verifiable clinical nursing experience.

n

nnPrior home health experience required.

n

nnOASIS experience required.

n

nnStrong clinical assessment and care coordination skills.

n

nnExcellent communication, interpersonal, organizational, and time-management skills.

n

nnAbility to work independently and effectively manage a field-based schedule.

n

nnExperience with electronic medical records (EMR) preferred.

n

nnReliable transportation and ability to travel throughout Monmouth, Middlesex, Union, and Somerset Counties .

nAdditional Information:

nBase Pay, depending upon qualifications, status, and subject to negotiation:

n$65.00 - $75.00 per point
