nVitality Hospice & Palliative Care is seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our clinician-owned and operated hospice agency. This is an excellent opportunity to work with a supportive interdisciplinary team dedicated to providing exceptional end-of-life care.

nKey Responsibilities
nnComplete comprehensive hospice admissions and patient/family assessments
nEnsure accurate documentation and appropriate care planning
nProvide direct patient care as needed, including medication administration and treatments
nCollaborate with physicians and the interdisciplinary team to develop individualized plans of care
nServe as a liaison between patients, families, and healthcare providers
nEducate patients and caregivers on disease progression, symptom management, and hospice services
n nQualifications
nnActive Registered Nurse (RN) license (required)
n1+ year of skilled nursing experience preferred
nHospice or admissions experience preferred (not required)
nWillingness to travel within assigned service areas
nStrong communication, organizational, and interpersonal skills
nValid driver's license, reliable transportation, and auto liability insurance
