Job description
Transitions Care is seeking an Admissions Registered Nurse to complete hospice and palliative care admissions, conduct comprehensive assessments, and educate patients and families about services and care plans. The RN will collaborate closely with an interdisciplinary team to coordinate symptom management, ensure smooth transitions from hospitals or facilities to home or hospice settings, and document accurately in the EMR. This role supports patients and families during serious illness and end-of-life, promoting comfort, dignity, and holistic, patient-centered care while upholding clinical excellence and regulatory standards.
