Job description
Transitions Care is seeking an Admissions Registered Nurse to complete hospice admissions and evaluations in patients’ homes and facilities. You will assess eligibility, explain hospice services, obtain consents, initiate care plans, and coordinate with our interdisciplinary team. The RN will manage symptoms, provide patient and family education, ensure accurate documentation, and support a smooth transition to hospice with compassion, cultural sensitivity, and clinical excellence.
