Job description
We are seeking a full-timeAdult Primary Care Physician to provide comprehensive medical care to membersenrolled in an Institutional Special Needs Plan (ISNP). This role offers theopportunity to make a meaningful impact on medically complex seniors throughproactive care management, interdisciplinary collaboration, and a strong focuson improving outcomes and preventing avoidable hospitalizations. Base Salary:$211,369–$317,053

Qualifications
MD or DO degree from anaccredited medical school.
Completion of an accreditedFamily Medicine, Internal Medicine, or Geriatrics residency program.
Board Certified or BoardEligible.
Active Nevada medicallicense, or ability to obtain licensure.
Valid DEA registration.
Two or more years ofexperience in SNF, LTC, post-acute care, or complex senior care preferred.
Experience with HCC coding,Medicare Advantage, HEDIS, and STARs documentation preferred.
Comfortable practicing ininstitutional and post-acute care settings.
Ability and willingness totravel between healthcare facilities in Nevada.
