Value-based Advance Practice Provider (NP /PA) managing chronic conditions of high-risk geriatric patient panel Monday-Friday. $10,000 signing package + benefits + quality-based bonus & salary range of $135k-$155k/yr.

Provide primary care to high acuity patients in their home Monday - Friday

Responsibilities
The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, and primary care. The APPs who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient's needs.
Qualifications Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
Federal DEA License/eligibility
Geriatric training/experience preferred
Active CPR/BLS Certification
Board Certification
Must maintain a valid driver's license and good driving record

USD $155,000.00/Yr.
Posted Min Pay Rate
USD $135,000.00/Yr.
