Advanced Case Manager - LPTC

Full Time
Join a dedicated team providing direct mental health services to preschool-aged children facing significant challenges in the Midwest region. As an Advanced Case Manager, you will play a vital role in delivering community-based, interactive care designed to enhance overall functioning and support children in the least restrictive environments possible.
Key Responsibilities
Deliver skill training aligned with measurable goals determined by a Qualified Mental Health Professional.
Provide intensive, strengths-based mental health services within the community.
Collaborate with families and treatment teams to develop and maintain client-centered treatment plans.
Support youth in understanding their symptoms and exploring solutions.
Facilitate connections with natural community supports and coordinate mental and physical healthcare services.
Offer crisis stabilization and conduct trauma-informed risk assessments under professional guidance.
Work closely with community partners and caregivers to ensure successful reintegration of clients.
Complete all required documentation accurately and promptly.
Schedule
Monday through Thursday, 10-hour shifts.
Education and Experience
Required: Bachelor's degree in Social Work, Psychology, or related field; OR Associate's degree plus 2+ years of relevant experience; OR equivalent combination of education and experience totaling 4+ years.
At least one year of experience working with children.
Preferred: Bachelor's degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics.
Three to five years of professional experience in mental health or related services is preferred.
Licensure and Training
Completion of CPST Case Management online training and other required screenings within 60 days of employment.
Valid driver's license, insurance, and reliable vehicle required.
