Loyola Medicine, a member of Trinity Health, is seeking an Advanced Practice Provider to join our Advanced Heart Failure Program, supporting patients with advanced heart failure, VADs, and heart transplants. This role combines inpatient and outpatient responsibilities within a growing program.
