Job Title: Adult-Gerontology Nurse Practitioner (AGNP)
Location: Escondido, CA
Schedule: 8:00AM - 5:00PM x 5
Term: 13-week assignment with the potential for extension
Pay Rate : $110 - $115/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Adult-Gerontology Nurse Practitioner (AGNP) for a temporary assignment in Escondido, CA. These positions are 13-week contracts with the ability to extend.
Responsibilities for a Adult-Gerontology Nurse Practitioner (AGNP):
Provide home-based hospice and palliative care as a Nurse Practitioner for patients with serious or life-limiting illnesses
Conduct patient assessments, develop individualized care plans, and manage symptoms
Perform face-to-face home visits, averaging 45–50 visits per month
Conduct goals-of-care discussions and advance care planning, including POLST and advance directives
Provide patient and family education regarding hospice and palliative care
Serve as a resource and support for home aides and the interdisciplinary team
Coordinate care with the interdisciplinary team and community physicians
Participate in IDT meetings and quality improvement initiatives
Collaborate closely with the Hospice & Palliative Care Medical Director
Document all patient encounters using My Unity EMR
Work Monday through Friday, with one Sunday per month (with a corresponding weekday off), 8:00 AM–5:00 PM
Participate in rotating weekends, occasional on-call coverage, and 24/7 patient coverage as needed
Maintain current California NP and RN licenses, DEA, NPI, MSN, AANP/ANCC certification, and BLS
Maintain a minimum of 3+ years of hospice or palliative care experience
Obtain ACHPN certification within one year of hire (preferred at hire)
Maintain a valid California driver’s license and auto insurance for local travel to patient homes
Spanish bilingual skills are a plus
