Job description
Monogram Health is seeking compassionate Advanced Practice Providers (NPs or PAs) to deliver in‑home, evidence‑based care to patients with multiple chronic conditions. You will work within an integrated multidisciplinary team and provide care in patients' homes and via telehealth, with a focus on population health and value‑based care.
You will conduct 30–90 minute visits, travel to patient homes, and participate in regular care team meetings.
