Job description
Piedmont Healthcare is recruiting an Advanced Practice Practitioner to join the Primary Care team at our outpatient practice in Sweetwater. The position is full time, Monday through Friday, with responsibilities across direct patient care and collaborative physician support.
The APP will focus on health promotion and chronic disease management, working with multiple IM providers to serve the PCP patient panel in an outpatient setting.
