Requirements: • Successful completion of NP or PA Graduate Program (Masters Level) • AANP or ANCC certification for Nurse Practitioners • NCCPA certification for Physician Assistants • Ability to obtain: Idaho and Oregon medical licensure, and valid controlled substance registration with the Idaho and Oregon Board of Pharmacy and DEA • 2 years' experience as a Sleep Medicine Provider preferred
