## Advanced Practice PractitionerApplylocations: Grand Rapids, MItime type: Full timeposted on: Posted Todayjob requisition id: JB **Summary of Essential Responsibilities**The APP will work closely with a physiatrist (a physician specializing in physical medicine and rehabilitation) to:* Perform acute care consultations for hospitalized patients.* Evaluate patients to determine the **most appropriate level of post-acute care,** such as: Inpatient rehabilitation, Skilled nursing facility, Home with therapy services, other rehabilitation settings* Assist the acute care rehabilitation specialist with inpatient rehabilitation consultations.* Collaborate with Mary Free Bed Rehabilitation Hospital physiatrists and the multidisciplinary care team to coordinate patient care.* Work primarily at Mary Free Bed Hospital while providing consultation services for patients admitted to Corewell Health's acute care hospital in Grand Rapids, Michigan.**Schedule*** Full-time* Monday through Friday* Weekend rotation of approximately 7-8 weekends per year**What this role requires**This position is well suited for a Nurse Practitioner (NP) or Physician Assistant (PA**)** with experience in:* Hospital medicine* Physical Medicine & Rehabilitation (PM&R)* Neurology* Trauma or orthopedic servicesThe work is generally consultative rather than procedural, with a focus on:* Comprehensive patient assessments* Functional evaluations* Discharge planning* Determining rehabilitation candidacy* Coordinating care with physicians, therapists, case managers, and nursing staffThis is a collaborative inpatient role centered on helping hospitalized patients transition to the rehabilitation setting that best matches their medical and functional needs.**Customer Service Responsibilities**Demonstrate excellent customer service and standards of behaviors as well as encourages, coaches, and monitors the same in team members. This individual should consistently promote teamwork and direct communication with co-workers and deal discretely and sensitively with confidential information.**Responsibilities in Quality Improvement**Contribute by identifying problems and seeking solutions.
