Job Summary and Responsibilities
nAs our Per Diem Nurse Practitioner/Physician Assistant, Family Medicine, you will be responsible for utilizing training and skills to provide direct patient care under the supervision of a physician. This position offers a strong work-life balance with weekday clinic hours and a shared phone call coverage schedule. We are looking for a true team player who is passionate about patient care and committed to providing exceptional customer service.
nEvery day, you will apply your comprehensive expertise to assess, diagnose, and treat a wide range of health conditions across various age groups, from pediatrics to geriatrics. This involves patient consultations, developing treatment plans, prescribing medications, and focusing on preventive care and health education, all within a collaborative health-care team.
nTo be successful in your role, you will meticulously provide direct patient care, utilizing your specialized training and skills to assess, diagnose, and manage a diverse patient population within family medicine, under physician supervision. You will demonstrate exceptional clinical proficiency and a compassionate approach, ensuring comprehensive care coordination and optimal patient outcomes within your scope of practice.
nnnProvide full-spectrum primary care, including preventive care, acute illness treatment, and management of chronic diseases (e.g., diabetes, hypertension) for patients of all ages

nnDeliver wellness examinations, health screenings, and immunizations to support disease prevention
n
nnCounsel patients on diet, lifestyle choices, and disease management Order, perform, and interpret diagnostic tests such as laboratory work and imaging
n
nnOffer guidance, counseling, and education on wellness, disease management, and medication adherence to patients and their families
n
nnAct as the first point of contact and coordinate care with specialists, nursing staff, and social workers
n
nnMaintain accurate, timely, and detailed medical records, utilizing EPIC appropriately

nJob Requirements
nRequired
nnnARNP Degree
n
nnCurrent State ARNP license with prescriptive authority.
n
nnCPR certification.
n
nnBCLS required.
n
nnCandidates must demonstrate a commitment to service, organization values and professionalism through appropriate conduct and demeanor at all times.

n nPreferred
nnOne year of previous APP experience in primary care, urgent care, or ED setting is preferred
