Advanced Practice Provider / Nurse Practitioner

Your Responsibilities Will Include

Conducting thorough patient evaluations, ordering necessary diagnostic tests, and developing personalized care strategies

Diagnosing and treating acute, episodic, or chronic health conditions in collaboration with physicians or independently (as per your credentials)

Educating patients and families regarding prevention, wellness, and management of diseases

Prescribing medication and therapy in alignment with your practice scope

Engaging in chronic care management and ensuring patient follow-ups

Working in tandem with interdisciplinary teams to guarantee comprehensive, patient-focused care

Maintaining accurate and detailed records within electronic medical systems (EMR)

Qualifications

Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)

Completion of an accredited Nurse Practitioner program

Essential Certifications

Active Nurse Practitioner licensure in Indiana

National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or relevant specialty

Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or willingness to attain within 90 days)
