Advanced Practice Provider - Palliative Care, Float

We are seeking an Advanced Practice Provider to join one of the nation's most integrated healthcare systems, with over 1,300 providers in Indianapolis, Indiana.
Community Physician Network seeks a Nurse Practitioner or Physician Assistant to join our Palliative Care team in Indianapolis, IN. The most comprehensive palliative care program of its kind in Indiana, our team is committed to helping patients and families understand where they are on their medical journey and providing the physical and emotional comfort they need.
0.6 FTE position, weekdays
This provider will work autonomously and collaboratively with our palliative care multi-disciplinary teams as a "float" provider at our network hospitals to deliver palliative and health care services to patients.
Focus on the comfort measures of patients, end of life care, symptom management, as well as assisting with consults and new admissions.
Flexibility to travel between hospitals in Indianapolis

Ideal candidates will:
Possess 1-3 years' Experience as a licensed Nurse Practitioner in Palliative Care
Be comfortable assessing patients independently
Be compassionate caregivers with excellent organizational, communication, and critical thinking skills
Possess highly developed interpersonal and group dynamic skills, as well as proven skills as a consultant/educator to friends, families, and loved ones
Educational background consistent with requirements for licensure and certification within the state of Indiana
