Hourly Pay Range:

$56.44 - $70.58 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Endeavor Health is continually recruiting nurse practitioners and physician assistants for future full-time and part-time positions in our Primary Care clinics across the Northwest Suburbs of Chicago. Position Highlights

Position: Advanced Practice Provider (APN/PA) - Primary Care- Family Medicine

Location: Various locations in the northwest suburbs (Arlington Heights, Rolling Meadows, Schaumburg, Mount Prospect, Buffalo Grove, Kildeer).

Full Time/Part Time: Full Time, 40 hours per week

Hours: Monday - Friday, hours vary

Saturday rotation and call rotation required

Job Description

Exciting opportunity to join Endeavor Health in thePrimary Care Department. Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes.Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures.

Qualifications:

License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required

Education: Completion of master's level program in specialty area

Certification: BLS/CPR certification required

Experience: Minimum of 1 year APN/PA experience in Primary Care setting required. 2 years of APN/PA experience in Primary Care setting preferred

For APNs, Family Nurse Practitioners only

Preferred Qualifications

Experience working in an outpatient family medicine setting

Experience with both acute and chronic condition management and preventative care
