Job Details:
Full-time Nurse Practitioner or Physician Assistant position supporting the home-based REACH (Resource for Elderly and Care at Home) Program
Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA)
Deliver compassionate geriatric care in patients' homes and assisted living facilities in the greater Manchester, CT area
Work within a multi-disciplinary care team focused on Geriatric Medicine in the home setting
Monday through Friday schedule with no weekend requirements
Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients
Develop plans for further evaluation of patient health needs, including referrals to specialists and other health team members
Provide appropriate treatment for problems within the scope of practice and refer appropriately when needed
Unrestricted State of Connecticut Medical License required (or eligible)
Minimum of two years of post-graduate experience as a Nurse Practitioner or Physician Assistant in Geriatric Medicine or a related field required
