Opportunity:
Advanced Practitioners - Primary Care
Job Details:
Full-time, outpatient primary care position focused on adult medicine (ages 18+) with a weekday-only schedule and no call required
Open to both Nurse Practitioners and Physician Assistants with 2+ years of experience in primary care or a relevant outpatient setting
Build your own patient panel in a patient-centered environment with ample support for seamless integration
Perform routine physical examinations and formulate medical, educational, and psychosocial treatment plans for patients
Develop plans for further evaluation of health needs in consultation with physicians and care team members, including referrals to specialists
Provide appropriate treatment for problems within scope of practice and refer appropriately when needed
Utilize state-of-the-art EPIC clinical and informatics technology to promote improved patient outcomes and streamlined continuity of care
Dedicated support and voice through the Office of Advanced Practice directed by system-level leadership
Access to robust professional development resources including CME, a Provider Leadership Development Institute, and an on-demand learning platform
Established mentorship and orientation programs specially designed for newly hired providers and those new to practice
Unrestricted State of Connecticut Medical License required (or eligible); APRN (Family, Adult/Geri Nurse Practitioner) or PA training required
