Opportunity:
Advanced Practitioners - Primary Care
Job Details:
Full-time Family Nurse Practitioner (FNP) or Physician Assistant (PA-C) position in a multi-provider primary care practice
100% outpatient primary care setting - Family Medicine, seeing patients from birth and up, no OB
Schedule is 4 days per week (Monday, Tuesday, Thursday, Friday) with no call required
Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients of all ages
Develop plans for further evaluation of health needs in consultation with physicians and care team members, including referrals to specialists
Provide appropriate treatment for problems within scope of practice and refer appropriately when needed
Utilize state-of-the-art clinical technology including EPIC and Ambient AI to promote improved patient outcomes and streamlined continuity of care
Dedicated support through the Office of Advanced Practice for Nurse Practitioners and Physician Assistants
Access to established Mentorship and Orientation programs for newly hired providers

Unrestricted State of Connecticut Medical License required (or eligible)
APRN (Family Nurse Practitioner) or PA training required
Two or more years of post-graduate experience in Primary Care seeing both pediatric and adult patients preferred
