This includes, but is not limited to, long term residential programs, halfway housing or sober living housing and outpatient treatment services. Responsibilities also include facilitating Community Support Services for clients and transporting.Essential Duties and Responsibilities:Responsible for assisting clinical staff with placement of clients in the appropriate facility as determined by their needs. Determines clients' eligibility for requested facilities.Works with long term residential facilities, halfway houses, and shelters to refer appropriate clients and secure placements.Maintains spreadsheet for LME of requested data regarding referral and placement of clients.Responsible for obtaining signed releases from the clients to allow the release of required information to the appropriate facilities/referrals.Documents all client contacts and community contacts into BUIResponsibility for transporting clientsFacilitates referrals to Community Service Providers including outpatient servicesResponsible for the maintenance and upkeep of all company vehicles as well as tracking mileage and maintaining mileage logs.Responsible to act as back-up in the absence of residential staffAll other duties as assigned by supervisor or other managementQualification RequirementsHighly motivated and resilient professionalDemonstrated leadership and supervisory experienceSome knowledge of or willing to learn clinical practice modelsDemonstrated ability to understand economic, business and operational factorsStrong critical-thinking/problem-solving skillsProven conflict management/resolution skillsHigh professional ethics and standardsStrong team player and team building skills; ability to collaborate with all levels and areasStrong presentation skillsAbility to effectively and positively influence and persuadeEducation and/or Experience: Must possess at least a High School Diploma. Prefer Bachelor's degree in related Social Work field with experience. Office experience in the healthcare field preferred.
