Gentiva is seeking an Afterhours RN Care Manager to deliver compassionate, clinically advanced home-based care to patients and families. In this role, you will provide afterhours triage, symptom management, and care coordination, ensuring timely, safe responses to patient needs. You will conduct assessments, implement care plans, collaborate with on-call providers, and document in the EHR. Working independently in the field, you’ll support patient dignity, comfort, and quality of life while contributing to a mission-driven, team-focused culture with strong clinical support and growth opportunities.
