Job description
Gentiva is seeking an Afterhours RN Care Manager to deliver compassionate, clinically sound home-based care to patients and families during evenings, nights, weekends, and holidays. In this role, you will provide phone triage, in-home visits, and care coordination, assessing patient status, managing symptoms, and implementing care plans in collaboration with the interdisciplinary team. You will educate patients and caregivers, ensure safe transitions, and support dignified, patient-centered care aligned with Gentiva’s mission of respect, empathy, and clinical excellence.
