Job description
Gentiva seeks an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours, ensuring seamless coverage and strong clinical oversight during nights, weekends, and on-call shifts.
You will assess, coordinate, and document patient and family needs, participate in the IDG, and manage care plans with physicians and staff. Must have RN license and ≥1 year hospice/home health experience.
