Job description
Gentiva Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours in Roanoke, VA. You will coordinate patient and family needs during nights, weekends, and on-call hours, while maintaining clinical oversight and guiding the interdisciplinary team.
Responsibilities include assessments, care planning with physicians, education for families, and ensuring compliance and documentation.
