Job description
Bloom Healthcare in Thornton, CO is seeking an On Call Hospice Nurse or LPN to provide and coordinate hospice care for patients and families after-hours, typically from home settings or community locations.
You will assess symptoms, educate families, document care, and collaborate with the interdisciplinary team to ensure comfort and dignity. A CO RN/LPN license and at least one year of relevant clinical experience are required. Competitive compensation and benefits are included.
