Gentiva is seeking an Afterhours RN Care Manager for our hospice team. In this role, you will provide compassionate triage, symptom management, and crisis intervention for hospice patients during evenings, nights, and weekends. You’ll conduct home visits, assess patient status, implement care plans, coordinate with the interdisciplinary team, and offer education and emotional support to families.
