After-Hours Hospice RN — Weeknight Shifts

Job description

VIA Health Partners, Fort Mill, SC, is seeking an After Hours RN for Weekday Nights to coordinate patient and caregiver care and conduct nursing assessments. You will make home visits, follow the plan of care, and adjust it as patient needs evolve.

Requirements include an active RN license (compact/home state), current CPR, and at least 2 years of direct care experience with life-limiting illnesses. CHPN and BSN are preferred.
