Job description
As an Afterhours RN Care Manager at Gentiva, you’ll provide clinically advanced, compassionate home-based care during evenings, nights, weekends, and holidays. You will triage patient calls, make urgent home visits, manage symptoms, coordinate with on-call providers, and update care plans to support comfort, dignity, and safety in the home. Collaborating with a mission-driven team, you’ll educate patients and families, document in the EHR, and ensure seamless transitions of care while upholding Gentiva’s standards of empathy, respect, and clinical excellence.
