Afterhours Hospice Care Manager RN (7/7 Schedule)

Job description

Gentiva seeks an Afterhours RN Care Manager to coordinate and deliver high-quality hospice care during nights, weekends, and on-call hours. You will assess patient needs, manage visits, and ensure continuous clinical oversight across Shelby, Jefferson & Saint Clair Counties.

You will participate in the IDG, support care planning, and educate families on hospice philosophy and symptom management. Strong EMR skills and leadership will help guide field staff. #J-18808-Ljbffr
