Job description
Gentiva is seeking an Afterhours RN Care Manager to provide compassionate hospice care outside standard hours, ensuring continuity of care and strong clinical oversight for patients and families.
You will assess needs, coordinate with the IDG, and support chart reviews, care plan updates, and regulatory compliance while delivering high-quality care in homes during nights, weekends, and on-call shifts.
