Job description
Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will perform after-hours visits, assessments, and care coordination to support patients and families during nights and weekends.
In this role you will maintain primary clinical oversight, collaborate with physicians and the IDG, educate families, and ensure accurate documentation while promoting compassion and professionalism in a fast-paced environment.
