Job description
Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. The role includes on-call, weekend, and night shifts with primary clinical oversight and active IDG participation to coordinate care for patients and families.
Responsibilities include comprehensive assessments, plan-of-care development, education on hospice principles, and timely documentation. A strong RN background in hospice or home health is required.
