Job description
Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will assess and coordinate needs during nights, weekends, and on-call shifts, with primary clinical oversight for assigned patients.
You will collaborate with physicians and the IDG, perform routine visits, admissions, and death visits, and ensure accurate documentation and compliance across the care continuum.
