Job description
Gentiva Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours, ensuring coverage and strong clinical oversight for patients and families during nights, weekends, and on-call shifts.
The role includes initial and ongoing assessments, care coordination with the IDG, planning of care with physicians, and education on symptom management, with emphasis on documentation, regulatory compliance, and compassionate leadership for field staff.
